RELEASE FORM

Fill out and submit prior to visiting us. This form should be filled out annually. Be sure that all attending are accounted for or another form will need to be filled out. A submitted form cannot be edited. Present ID to recall your release form upon your visit. The release form is all online, there is no need to print.

Each renter must be 18 years old or have a

parent/guardian signature on our release form.

A valid phone number MUST accompany

signature for phone call verification for any minors.
